Medical errors (MEs) are one of the top causes of death in the United States, represent one of the leading causes of preventable harm to patients, and cost in excess of $17 billion annually. To prevent MEs, they must be identified, reported, openly discussed, and learned from; otherwise, they will likely be repeated. This panel symposium includes a discussion of current directions in research and practice in the area of ME management and prevention. Panelists will discuss opportunities, challenges, and strengths unique to conducting research on errors in applied medical settings, as well as practical aspects of applying that research to patient care environments.
